Forensic Medical Examination Report of Urgent Risk Assessment of Gender-Based Violence

Incident descriptions, previous incidents (including unreported ones), court receiving requests (contact data, person who received the protection order request), past history of violence (sexual /physical assault and/o threats, domestic violence, violence in the community), background of violence against partner (previous physical abuse, sexual abuse and extreme jealousy, use of weapons and/or death threats, recent pattern or escalation of violence), previous breach of court disposals, negation, contempt, justification of violent behaviour, assessment of the current assault (the motive for current forensic medical risk assessment), serious physical or sexual assault, use weapon or death threats, breach the exclusion order.
